The boat I recently got in trade came with a 2004' Johnson 3.5hp 2 stroke. The guy said he has had it since '06 and had barely used it. He fired it up for me briefly.

Is there anything I should do just incase? New plug? Fuel additive?

It is a very clean motor, looks brand new.

if it has a water pump ?it has to be changed , not a hard job , change oil in lower unit. mix up some new gas and some sea foam, go fishin. the water pump is a must. you,ll burn the motor up before you noticed it quit working.
